Appointment intake form
This form was recently updated to include everything from personal & medical information to information about the services you have booked. Please have this filled out before your appointment. Read the important information below pertaining to your service.
For massage
- I no longer offer pregnancy massage, but I can refer you to someone who does
- Please don't shave or wax right before your service. Your skin could get irritated.
- If you have any serious medical issues, please check first with your doctor
- For face massage, no botox, fillers, etc at least 1 to 2 weeks before.
- Please let me know in forms, if you have any issues with lying on your stomach.
- Please wait at least 1 to 2 weeks after a piercing or tattoo to get massaged.
For facials, Peels & Body treatments
- No shaving or waxing a few days prior to facial
- No botox, fillers, etc at least 1 to 2 weeks before facial
- No peels at least 2 weeks before facial
- Discontinue using Vitamin A (Retinol, Trentinoin, etc) at least 3 days before facial
- Please wait at least 2 weeks after a piercing or tattoo.
For Peels:
- Discontinue use of products containing salicylic, glycolic, retinoids and other exfoliants at least a week before your peel.
- No botox or fillers, at least 1 to 2 weeks before peel
- Avoid shaving or waxing at least a few days prior to peel
- Please wait 2 weeks after a piercing or tattoo.
